MetLife is the County’s dental carrier effective July 1, 2008. You may elect coverage under one of the dental plans offered by MetLife whether or not you participate in a County medical option. MetLife Preferred Dentist Program (PDP) is the name of the participating network of dentists. However, you will not be required to use a participating network dentist to receive benefits - you may see any dentist you choose. You should be aware that if you see a non-network dentist, the plan will only pay up to the Reasonable and Customary (R&C) fee for services rendered.
MetLife does not issue dental ID cards. All you need to do is provide your social security number to the dentist and he/she can verify coverage by contacting MetLife at 1-800-438-6388. You may also print dental claim form (below) to present to your in-network or non-network dentist.
